You have access to EAP forms online
As a clinician, you focus on providing the best care possible to those you serve. Magellan’s goal is to support you by making the essential tools and care documentation process easy to navigate—so you can concentrate on what matters most: helping those who rely on your expertise.
You will find program-specific forms in the member’s registration packet. In most cases, Magellan does not send EAP member registration packets by mail. Providers can view and print the packet documents securely online. Read more.
However, you also have easy access to generic EAP Forms on our provider website. Bookmark this page for when you need it!
Two forms we’d like to highlight are the Statement of Understanding and the Follow-up Summary.
Statement of Understanding (SOU)
The SOU form helps members understand the services available to them while under your care, including assessment, counseling, and referrals, while setting clear expectations of the EAP’s limits. Recent audits found that 48.5% of charts included a completed SOU.
You have access to the generic versions of the SOU in English (PDF) and Spanish (PDF), as well as the version of the SOU for Federal Occupational Health (FOH, PDF).
Follow-Up Summary
While Magellan’s Follow-Up Summary (PDF) form isn’t required, we encourage its use to help ensure all necessary details are documented. Our audits show that providers are doing well with case outcome documentation (77.2%), but there is a slight decrease (68.8%) in documenting follow-up efforts. Whether you choose to use the Magellan form or simply include the key information in the chart, documenting case outcome details—including referrals when applicable—helps reflect the clinical decisions you’ve made. We’re here to make the documentation process simple and straightforward.
