Requesting preauthorization for injectable medications

For Blue Shield of California and Sharp Health Plan members

Each health plan has different preauthorization requirements for injectable medications.

The plan will review requests for medical necessity and notify the provider and member of the determination. The health plan will also notify Magellan of the prior authorization request and determination.

Blue Shield of California

To request authorization for an injectable medication, review information about Obtaining In-Office Injectable Medications (PDF), then complete the Prescription Drug Prior Authorization or Step Therapy Exception Request Form (PDF). Fax the form to 1-844-262-5611.

Sharp Health Plan

To request authorization for an injectable or infusion medication, complete the Behavioral Health Injectable/Infusion Prior Authorization Request Form (PDF). Fax the form along with applicable medical records to Sharp Health Plan at 1-858-636-2426.
