EAP providers: The new, enhanced online EASI Form is here
Magellan is enhancing provider portal applications to make it easier for providers to complete essential tasks securely.
One such task? Requesting reimbursement for EAP services. Now you have access to the enhanced online EASI Form.
- Sign in to MagellanProvider.com.
- Select Submit an EASI Form from the left-hand menu.
- You then will be directed to register/link your new account with multifactor authentication. This extra step enables you to access the new online EASI Form.

What hasn't changed
The information you enter and the timeframe for submitting the EASI Form remain the same.
What's next
You can bookmark the new EASI Form or continue to access it through MagellanProvider.com—it’s up to you once you link your existing MagellanProvider.com account with the new environment.
You also will have an opportunity to share feedback about your experience with the new EASI Form. This is the first application to move to a new platform, so you’ll get an inside look into the new, user-friendly format. We will use your feedback as we develop additional provider portal applications.
